Universal Health announces the opening of a new outpatient facility to address the gap in healthcare between a patient’s discharge and their return to home. Universal Transitional Care Center is now taking appointments in and will serve the Roanoke Valley and New River Valley.
“This is something we’ve been planning for a while, and we’re eager to see it play an active role in the area’s healthcare ecosystem,” said Joseph Park, VP of Business Development. “This center will target the people who are at risk of being overlooked while they navigate the path back to everyday life following time in a hospital, rehab center or nursing home.”
All too often, said Park, patients face difficulty scheduling time with their primary care physician once they’ve been discharged. While they no longer require daily care and monitoring, it can be premature to send them home with no further attention. Some patients continue to need additional support from home health, a skilled nursing or rehabilitation center or a long-term care facility.
It’s not uncommon for someone to wait weeks to see their primary care physician following an acute stay. Universal Health Transitional Care Center focuses on that 30- to 60-day window after an acute stay. The clinic will initially be open Tuesdays and Thursdays from 8 a.m.-5 p.m. and will eventually expand to Monday through Friday. Patients will have access to the clinic within 48 hours of their discharge.
Park added, “This is the first step in a planned expansion into a void Universal Health has identified. Once our Roanoke location finds its footing and meets with some success, we can move forward with other, more remote locations.”
Universal Transitional Care Center is located at 1111 S. Jefferson St., Suite B in Roanoke. The center will provide wellness checks, medication management, wound evaluations, behavioral change assessments, and more.
